Which occupational disease is caused by extreme cold climate?

A 58-year-old man presents with irritability progressing to confusion and altered sensorium over 2 days. He has a known history of chronic liver disease along with mild renal impairment. His blood glucose level is normal. Which clinical feature MOST strongly supports a diagnosis of hepatic encephalopathy?

Given below are two statements: Assertion (A): Progression from confusion to coma in hepatic encephalopathy is usually sudden without preceding behavioral changes. Reason (R): Metabolic encephalopathies often show a stepwise progressive deterioration of cortical function. In light of the above statements, choose the correct option:

Match the clinical condition with its recommended dietary intervention: Conditions1. High-grade fever2. Low-grade fever3. Prolonged fever4. Recovery phase Dietary Requirement A. High-protein, high-calorie diet B. Easily digestible liquid and semisolid diet C. Gradual return to a normal balanced diet D. Increased hydration with fluid and electrolyte restoration

Match the Hypertension Dietary Modification with its underlying Mechanism/Principle: Dietary Modification1. Sodium restriction2. Potassium-rich foods3. Saturated fat reduction4. Magnesium intake Mechanism/Principle A. Improves vascular smooth muscle relaxation B. Reduces plasma volume expansion C. Improves endothelial function and lipid profile D. Enhances natriuresis and blood pressure control

Match each therapeutic diet with its principal physiological or biochemical effect: Therapeutic Diet a. Ketogenic diet b. Low sodium diet c. High fiber diet d. Low purine diet Physiological/Biochemical Effect1. Promotes ketone body production for neurological stability2. Reduces extracellular fluid volume and blood pressure3. Enhances bowel motility and modulates glucose absorption4. Decreases uric acid synthesis Select the correct match:

A nephrologist designs a diet plan for a patient with advanced chronic kidney disease (CKD stage 4). Consider the following statements regarding dietary management: 1. Protein intake should be restricted but must meet essential amino acid requirements. 2. Sodium restriction is beneficial in controlling edema and hypertension. 3. Potassium intake should be restricted only if hyperkalemia is present. 4. Fluid restriction is universally required irrespective of urine output. Which of the statements given above are correct?

A 70-year-old bedridden patient with a history of ischemic stroke presents with persistent dysphagia for 2 months. Despite speech therapy, swallowing function has not improved. The patient has experienced multiple aspiration events and significant weight loss. Gastrointestinal function remains intact. Considering long-term nutritional goals, complication prevention, and patient comfort, what is the MOST appropriate feeding strategy?

A severely malnourished patient with BMI < 16 kg/m2 is admitted and planned for nutritional rehabilitation. The clinician is cautious about refeeding syndrome. Arrange the correct sequence of management steps: 1. Initiation of low-calorie feeding (approximately 10 kcal/kg/day) 2. Correction of electrolyte imbalances, especially phosphate, potassium, and magnesium 3. Gradual escalation of caloric intake over 4–7 days 4. Continuous monitoring of vitals, ECG, and biochemical markers
